WAIVER OF LIABILITY

Parent/Guardian is solely responsible for their own dependent(s)’ safety and assumes any and all liability and risks of harm associated with or arising out of participation in activities at The Opportunity Center at ALCC and/or while participating in off-site programming provided in connection with The Opportunity Center at ALCC. Parent/Guardian also agrees to indemnify and release and hold harmless MCOP, its officers, agents, employees, insurers, and affiliates from any and all liability whatsoever arising out of or resulting from their dependent(s)’ participation in activities at/associated with The Opportunity Center at ALCC. Parent/Guardian agrees that their dependent(s) understand and will comply with all rules imposed by MCOP and The Opportunity Center at ALCC, and will conduct themselves in a reasonable manner at all times. Parent/Guardian understands that participation in physical activities involves the risk of serious injury. Furthermore, MCOP is not responsible for any property that is lost, damaged, or stolen while in, on, or around the premises.
Acknowledgement Of Mask/Face Covering Mandate:
Participants & Parents Inside The Opportunity Center at ALCC must have a face covering.

PARTICIPATION AGREEMENT

Everyone who participates in programs provided by The Opportunity Center at ALCC is expected to treat the facility and all fellow participants with dignity and respect. By signing below, you acknowledge you and your dependent(s) understand and agree to all of the expectations listed below:
• Participants engaging in activities provided by The Opportunity Center do so at their own risk. By signing this form, parents/guardians acknowledge they believe their dependent(s) to be in good enough health to participate.
• Alcohol, illegal drugs, or any other intoxicating substances are strictly prohibited.
• Good sportsmanship is expected of all participants. Name-calling, fighting, swearing, and other aggressive or intimidating behaviors will not be tolerated.
• In the event of an emergency medical situation, parents/guardians understand that MCOP staff will first attempt to contact the parents/guardians, and further agree to allow MCOP staff to secure emergency medical treatment for their dependent(s) when deemed necessary.
• MCOP is an equal opportunity provider and is prohibited from discriminating based on race, color, national origin, sex, religious creed, disability, age, political beliefs, or reprisal or retaliation for prior civil rights activity in any programs. Discrimination or other intolerance of any kind will not be tolerated.
• Any photos taken of activities at or associated with The Opportunity Center may be used for promotional purposes.
• All participant information collected by MCOP will remain confidential and will only be used for the benefit of participants, including tracking demographic trends, service patterns, and outcomes achieved for reporting purposes to our funders and partners. MCOP will retain information in accordance with its Document Destruction Policy.
